I, SRI PARTHA HAZRA, son of MukundaHazra, aged about 35 years, by faith Hindu, by occupation- Business, residing at N0223, Bauria Station Road, near Bauria Girls High School H.S., East Burikhali, Bauria, P.O.- Burikhali, P.S.Bauria, District- Howrah, Pin- 711310, do hereby solemnly affirm on oath and declare as follows:-

1.ThatIam Indian National by birth.

2.That my son’s name is KRIPYANSH HAZRA who was born on 08/04/2022.

3.That in the Birth Certificate of my son issued by Government of West Bengal, Depertment of Heaith and Family Welfare, MunicipaliltyUluberia, my son’s name has been recorded as KRIPYANSH HAZRA.

4.That due to astrological ground now I want to change my son’s name as KRIPYAANSHU HAZRA from KRIPYASH HAZRA.

5. That henceforth, my son will be recognized KRIPYAANSHU HAZRA.
6. That KRIPYAANSHU HAZRA and KRIPYANSH HAZRA both are same and one identical person.

7.That the purpose of this Affidavit is to avoid any confusion regarding my son’s names.

8.That the stetments made above are true to the best of my knowledge.     94/16
